Incorporating Patient and Donor Choice in Kidney Exchanges

Overview

Kidney exchange programs are recent initiatives that allow patients who are waiting for a kidney transplant to find a willing but incompatible donor, to swap with another patient's donor. Based on various factors such as the donor's age, donor's health condition, patient's location, etc., the patients and donors involved in the swap may choose whether or not to proceed with the transplant, a process which we call patient and donor choice. In this paper, we make two major contributions. First, we improve the current model of the kidney exchange program, which is modeled as a graph with the patient-donor pairs and altruistic donors as vertices and potential transplants as edges. We introduce fuzziness into the vertices and edges, increasing the expected utility of the swap. Furthermore, while it has until now been restricted to kidney allocation, we extend existing research on patient and donor choice to kidney exchanges and present two methods to optimally implement it.
